A poster developed by Northampton General Hospital NHS Trust demonstrates, in an accessible way, the application of a range of improvement techniques and tools to deliver the ambition to reduce the incidence of pressure ulcer grades 2-4.
Tools and techniques include:
- the use of a pareto chart to identify the most problematic areas
- fishbone diagram to make the system linkages that are essential to tackle towards reducing incidence of avoidable pressure ulcers
- PDSA cycles applied to the design of waterlow risk assessments, bodymap forms, matron walkround forms and the sustainability model to identify the key barriers to embedding the changes to secure sustainable improvements
