Keeping patients safe when they transfer between care providers: getting the medicines right
This final report by the Royal Pharmaceutical Society (RPS) outlines the results of a six month project involving over 30 healthcare organisations and multidisciplinary teams that volunteered to implement RPS guidance on transfer of medicines information.
The aim of the guide was to recommend how best to support the commissioning and provision of services involving the transfer of information about medicines. The guidance describes lessons from the early adopter sites, core principles for the transfer of information on medicines, recommended core content of records, organisational guidance and fact-sheets.
